How do I get this addressed as its a breach of the court undertakings, what do I need to do ?

Thanks

Have you reported any of this to the police?

I assume kids are involved in this. If this happening during handovers then you need to meet somewhere else neutral . this will then make it harder for ex partner to be hostile. also you have to look at situation and try and find ways to minimise conflict. if they are that bad maybe see if you can get someone else to do handovers until it calms down. there are many things you can do but it depends on what contact you got and age of child etc. for example going to school is a good one for handovers.

Problem with police its very hard to do anything without evidence. unfortunately also if they get involved your ex could suggest its you and not herself which makes things more complicated. unfortunately also its hard for dads to defend themselves once ex starts turning things around which can lead to lots more complications. its all about looking at your situation and putting steps in place to minimise confict and hostility between you and ex. Maybe communicating solely through WhatsApp/email and not in person etc

this is very true, and I am not really sure how to handle it, I keep meaning to voice record the incident but then in the heat of the moment I never manage to. She has denied incidents before and I feel like I need to be physically and visible hurt to have any proof.

I find it hard that the police can't do anything as this is domestic abuse, but you probably are going to need to record the incidents to get anywhere.
